New Periodontal Patient Information – Louisville, KY
Beginning Your Cordini Periodontal Experience
Whether you’ve been referred by your regular dentist or have requested a consultation on your own, Dr. Cordini is excited to be able to begin working with you so that you can regain the quality of your life. Please reach out to us if there’s anything you want to clarify before your appointment. The information on this page should help give you some important context about your initial visit as far as what you can expect.
Your First Visit
At your first appointment, we’ll ask you a series of questions about the current state of your oral health – any concerns you might have, any problems you’ve noticed recently, and anything significant in your oral health history. We’ll take the necessary pictures and 3D jaw scans of your mouth and use them to help you formulate a blue-print or treatment plan. Once you decide on your optimal plan we will help you achieve your goal!
New Patient Forms
When visiting any dental office for the first time, you’ll have to fill out some paperwork so that we have your essential information on file. We’ll need to make sure everything is in order before moving forward with your first consultation. We recommend filling out the required forms ahead of time so that they’re ready to go by the time you get to our periodontal office, cutting down on the amount of time you spend waiting.
Dental Insurance Welcome
Dental insurance will usually help pay for common periodontal procedures. Your plan may or may not cover implants, but you can usually expect to receive benefits for treatments that are considered to directly benefit your oral health.
Your dental visits with us are usually reimbursed by your flexible spending accounts (FSAs), health reimbursement accounts (HRAs), and health savings accounts (HSAs). If requested, we can provide you with a statement of services to present to your administrator as reimbursement may be sought through the FSA claim submission process.
Financing Available
If you’re not sure whether you’ll be able to afford your care up front, you can ask us about financing options. We can help you apply for a CareCredit or Proceed finance plan, breaking up the price of treatment into a series of payments so that you can fit it into your budget more easily. The links below will let you apply for a plan right away, but you can always call us if you have any questions about the process or your options.
